At Southside, we want to make it easy for you to find the forms and information you need. Below, you will find forms for various church ministries, activities, and family requests. Building Use & Rental Agreement Form
A completed Building Use and Rental Agreement Form is required for anyone requesting to use Southside Baptist Church facilities. This form must be submitted before the scheduled event to ensure proper scheduling, approval, and understanding of the church’s facility-use guidelines.
STUDENT MEDICAL AND LIABILITY RELEASE FORM
A completed Student Medical Release Form is required for all students participating in off-campus church activities. Each form is valid for one calendar year and must be renewed annually to ensure we have up-to-date medical and emergency contact information.
